RAWE – Protection Of Personal Data Information Request Form

GENERAL

In order to evaluate and resolve applications to be made by you, data owners, in accordance with Articles 11 and 13 of the Personal Data Protection Law No. 6698 (“KVKK”) promptly, effectively and comprehensively, this Application Form has been prepared by Rawe Moda Tekstil Giyim Sanayi ve Dış Ticaret A.Ş. in its capacity as data controller.

APPLICATION METHOD

You, data owners, in accordance with Articles 11 and 13 of the KVKK; can submit your requests regarding the implementation of the KVKK to our Company, which has the title of data controller, in writing by filling out this form or by other methods determined by the Board:

• By applying in person to the address “Alsancak Mahallesi Atatürk Caddesi No:368 İç Kapı No:14 Konak İZMİR” with a signed copy of this data owner application form or by sending it via notary public, you can send a signed copy of this data owner application form to the ………………. e-mail address by using the electronic mail address previously notified to the data controller by the data subject and registered in the data controller's system.

DATA SUBJECT'S REQUESTS

In order to ensure the security of your personal data, within seven (7) days from the date your request for information reaches our COMPANY, our COMPANY may contact you to confirm that you are the data subject and may request some information and documents from you in this regard. The information and documents you provide to us within this scope will be immediately destroyed after confirming that you are the data subject.

In case the requested information and documents are incomplete, the information and documents must be completed and submitted to us upon our request. The thirty (30) day period specified in Article 13/2 of the KVKK regarding the finalization of the request will be suspended until the information and documents are completely submitted to us.

I request that my information request made in accordance with the KVKK be evaluated and finalized within the framework of the request/requests I have specified above, and I accept, declare, and undertake that the information and documents I have provided to you in this application are true, current, and belong to me.

I allow your Office to process the information and documents I have provided in this application form, limited to the purposes of evaluating, answering my application made in accordance with Article 13 of the Personal Data Protection Law No. 6698, delivering my application to me, and determining my identity and address.
